Donald Trump's visit to the UK was never going to pass without incident. But in the brief time the US President has spent here he has managed to step in front of the Queen, awkwardly hold hands with Theresa May - twice - and insult her entire Brexit plan. Her Majesty was forced to stand on her own on the raised dais in the Quadrangle of Windsor Castle.
